This serves as official notice that Gaston County Department of Health and Human Services-Social Services Division is soliciting and will receive proposals for In-Home Aide (IHA) Services as outlined in the following specifications. Gaston County DSS is issuing this RFP to secure In-Home Aide Service Provider(s). This Request for Proposals (RFP) process is the means by which Gaston County DSS will determine which Service Provider(s) are most qualified to meet the IHA needs of Gaston County DSS' clients. The qualifications must include specific information, as defined below. Gaston County DSS reserves the right to make awards to one or more vendors as best meets the agency's needs. Gaston County DSS does not guarantee any minimum amount of services. Any contract award is contingent upon the availability of funds to purchase specified services. In-Home Aide Services: a. Level I- Home Management In-Home Aide Services at this level are intended to provide support to individuals/families that require assistance with basic home management tasks, such as shopping, housekeeping, cooking, and bill paying. b. Level II- Personal Care In-Home Aide Services at this level are intended to provide support to individuals/families that require assistance with basic activities of daily living. Assistance with basic personal care tasks can be provided to the client when his/her capabilities are diminishing or when the client is striving to maintain or improve his own functioning. c. Level Ill- Personal Care In-Home Aide Services at this level are intended to provide substantial activities of daily living support to clients who require assistance with health/personal care tasks. Provision of these tasks involves extensive "hands on" care and potential assistance with a wide range of health-related conditions.
