Scope of work	Proposals for the provision of In-Home Aide Services (Levels II, and III) for older adults in Yadkin County for State Fiscal Year 2019 (July 1, 2018 – June 30, 2019) will be received by Yadkin County until 2:00 p.m. on Friday, May 4, 2018. These services are funded through the Home and Community Care Block Grant (HCCBG). The Piedmont Triad Regional Council Area Agency on Aging will serve as the administrative agent and all proposals will be reviewed by the Yadkin County Aging Planning Committee. The Request for Service Proposal package will be available after 12:00 noon on Friday, April 20, 2018. The package may be picked up at the County Manager’s Office, Human Services Building, obtained on the county’s website (www.yadkincountync.gov) or you may call 336-849-7911. A recommended HCCBG Request for Service Proposal requirements session will be held at 2:00 p.m Friday, April 27, 2018 at the Human Resources Building. Yadkin County reserves the right to reject any and all proposals or to waive any or all formalities. Current unit costs and seniors served for In-Home Services according to Blair Barton-Percival with the Piedmont Triad Regional Council Area Agency on Aging are: Level II: $22.3726 19 Level III: $24.0449 04 In addition, the Piedmont Triad Regional Council Area Agency on Aging states that there is $97,322 available In Home and Community Care Block Grant Funds with a required 10% local match of $10,814. Funding is subject to change depending upon availability of federal and state dollars.
